 If I schedule an appointment what is the cancellation policy?

ANS. There is a deposit of $100 collected to hold appointment for the fist time and which is used towards the visit when the patient comes in. You have 48 hours to cancel for full refund of that deposit.  However, if the patient reschedules the appointment, that deposit will still hold for the new date



What should I bring with me to my appointment?


Any recent blood work, xrays and other procedures (within the last 3-6months) would help to avoid unnecessary duplication



What are the office hours?


Brooklyn (EST)

Mondays - 10am - 5pm

Tuesdays - 9am - 6pm

Wednesdays 11am - 4pm

Thursdays - 10am - 6pm

Fridays - 10am - 4pm


Los Angeles (PDT)

Mondays, Tuesdays, Thursdays and Fridays - 11am - 5pm (EST)

Wednesdays - 11am - 4pm



What if I need to see the doctor and the office is closed?


Depending on the need, most times we can schedule a phone consult OR contact Dr Kokayi via email OR the patient might need to go to the Emergency Room in ACUTE cases



What are the policies for fees and payments?


Dr Kokayi does NOT accept any insurance so the patient pays in full at the time of service and we provide an insurance claim form to the patient to apply for reimbursement from their insurance if they have out-of-network coverage (It is the responsibility of the patient to check with their insurance for coverage details)



Who do i call if I have a billing question?


Dr. Kokayi's assistant is available Monday-Friday to help with any billing questions. 



What do I do if I run out of medicine or medications?


Call the office and the assistant will help you. With the supplements, she can arrange for you to pick up from the office, order them for you or ship them to any address you provide within the U.S



Do you accept insurance?


No, but we will provide you with a claim form to send to your insurance for reimbursement (the patient is responsible to check with their insurance for coverage details)
